Other; The sample consists exclusively of oncocytic cells.

Note: In a patient with follicular nodular disease, the findings likely represent a oncocytic cell hyperplasia, but a oncocytic cell neoplasm cannot be entirely excluded. Clinical correlation is advised.

Final diagnosis: Oncocytic adenoma

Nuclear atypia;  A few follicular cells with subtle nuclear atypia

Note: 대부분은 hemosiderin laden macrophage로써 adenomatous hyperplasia에 동반된 change로 생각되지만, 함께 도말된 소수의 follicular cell의 핵의 atypia가 약하게 있고 단단한 결집력을 보여 NIFTP에 cystic change가 있는 경우를 배제할 수 없습니다.  임상적인 소견을 참고하셔서 진단하시길 바랍니다.

Follow up cytology; Suspicious for malignancy (TBSRTC Category V)
- Suspicious for papillary carcinoma in the backgound of hemorrahgic cystic smear

Nuclear atypia; Relatively moderate cellularity follicular cells; some show subtle nuclar atypia and focal architectural atypia. 

Note: The possibility of Follicular neoplasm or NIFTP should be ruled out. A repeat aspirate after an appropriate interval of observation may be helpful if clinically indicated.

Note:대부분 세포는 benign follicular cell로 구성되어 있으나 일부 cell cluster가 nuclear enlargement, pale chromatin, and irregular nuclear contour를 보여 악성의 가능성을 배제할 수 없습니다. 조직학적 검사로 확인하는 것이 좋겠습니다.

Other; 
Microcalcific debris with few multinucleated giant cells and sparse degenerated follicular cells are seen.
Note: Microcalcific debris mimicking psammomatous calcification are associated with both benign and malignant thyroid aspirates including papillary thyroid carcinoma.
Clinical and radiologic correlation and follow-up are recommended.

Follow up cytology; Benign 
Suggestive of granulomatous (subacute) thyroiditis see note

Note: Benign follicular cell clusters, Lymphocytes, mucltinucleated giant cell로 구성된 smear 소견입니다. 외부슬라이드에서 언급된 psammoma body는 관찰되지 않고 colloid aggregates로 생각되는 물질이 관찰되어, 상기 smear 소견을 고려하면 granulomatous (subacute) thyroiditis의 가능성이 높을 것으로 생각합니다.
